Ultimately, the last failure was hydraulic related.
When I removed the burnt slave that CM gave me (wrong stoke BTW), the small rubber top hat stayed inside the Plastic Tee fitting that couples the rubber line to the hard line of the slave.
When I installed my stock OEM slave, it had it's own rubber seal and that stacked up on top of the lodged one from the CM slave. They essentially created their own little check valve. When I pumped up the slave and released the pedal, the master was pulling air into the system, then the next pump would extend the slave a bit, then draw in more air.
It's hard to visualize this without having as much facetime with these parts as I have. Needless to say, the slave failed within the first five pumps, even before I started the car. It still performed OK but the seal was breached.
So, I installed the new correct slave, and very meticulously bled the system, and so far it is operating as it should.
BOTTOM LINE: CM's fly, clutch (2nd one) and pressure plate were fine, however if they had sent me the correct slave, the 2nd install would have been the end.
I'm gonna take the fault for doubling up that little seal the 3rd time, but there wouldn't have been a third time if the slave was right.
tl;dr-fixed my stage 4 clutch issues.
